  • Shield Wise This plan offers a $2,500, $3,500 & $4,500 deductible All benefits accrue toward deductible and then are paid at 75% for the $2,500 deductible, 65% for the $3,500 deductible, and 55% for the $4,500 deductible until maximum out of pocket is reached. At that point, all plans pay 100%. $25 copayment for the two office visits per calendar year for the $2,500 health insurance plan, $35 copay for the $3,500 plan & $45 copay for the $4,500 plan. Further visits can help you to reduce the deductible on your California health insurance plan. For generic prescriptions, there is a $10 copay and no deductible, and for brand name prescriptions, there is a $35 copay & a $3,000 deductible.
  • Shield Secure This plan offers $2,070, $4,000 & $6,000 deductibles. All benefits accrue toward deductible and then are paid at 60% until maximum out of pocket is reached, at which point they will be paid at 100%. There is a $30 copayment for physician visits with no deductible. For generic prescriptions, there is a $10 copay and no deductible, and for brand name prescriptions, there is a $35 copay & a $3,000 deductible.
  • Shield Secure Plus This plan offers $2,070, $4,000 & $6,000 deductibles. All benefits accrue toward deductible and then are paid at 70% until maximum out of pocket is reached, at which point they will be paid at 100%. There is a $30 copayment for physician visits, with the deductible waived. There is also a $10 copay for generic prescriptions, with the deductible waived, or a $35 copay & $5,000 deductible for brand name prescriptions.
