The different tiers also have various levels of maximum out of pocket expenses and deductibles.  The higher the tier level, the lower the out of pocket and deductible, while the lower the tier, the higher the out of pocket and deductible amount.  This is inversely proportionate to the monthly premium.  The basic idea is that if you pay more per month, the insurance will pay more when you visit the doctor, and, if you pay less, then the insurance will also pay less.
