Medicare Supplement (Medigap) Plans California
Medicare Supplement (Medigap) plans are a great option for those who want both the freedom to choose their providers and the predictability of limited out-of-pocket costs.
With a Supplement:
- You can see any provider nationwide who accepts Medicare.
- You don’t need to worry about whether your doctor is contracted with a specific insurance carrier.
- The key is whether the provider accepts Original Medicare, since Medicare remains your primary coverage.
Provider Freedom, Medicare Assignment & Excess Charges
The vast majority of Medicare clinicians accept assignment, meaning they agree to accept the Medicare-approved amount as full payment for covered services. With providers who accept assignment, Plans F, G and N offer similar coverage after accounting for the Part B deductible that applies to Plans G and N, plus Plan N’s copays of up to $20 for certain office visits and up to $50 for certain emergency room visits.
Approximately 2% of Medicare providers do not accept assignment. For certain covered Part B services, they may charge up to 15% above the Medicare-approved amount (Part B excess charges). Plans F and G cover eligible excess charges; Plan N does not. That makes Plan N a potentially significant premium-saving option, but it is worth checking whether your preferred providers accept assignment. Benefit Packages can help you check.
Unlike a Medicare Advantage PPO, where an out-of-network provider may decline to treat you even if the plan offers out-of-network benefits, Original Medicare with a Supplement lets you see Medicare-participating providers nationwide who are accepting new Medicare patients. You do not need to worry about whether they contract with your Medicare Supplement carrier because Original Medicare remains your primary coverage. Providers who have opted out of Medicare are a different case.
Medicare Supplement Plan G
Medicare Supplement Plan G is one of the most comprehensive options available to people who are new to Medicare. It covers most out-of-pocket expenses that Original Medicare doesn’t pay, except for the annual Medicare Part B deductible.
Plan G also covers Part B excess charges, helping protect you from additional costs when you see Medicare providers who do not accept assignment.
Plan F vs. Plan G
Plan F also covers excess charges, but it is only available to individuals who were eligible for Medicare before January 1, 2020.
For those currently enrolled in Plan F, we often recommend reviewing a switch to Plan G because:
- The primary benefit difference is that Plan G does not cover the annual Part B deductible.
- The 2026 Part B deductible is $283.
- In many cases, the premium savings with Plan G may exceed that deductible.
Medicare Supplement Plan G vs. Plan N: Is the Higher Premium Worth It?
While Plan G offers comprehensive coverage, Medicare Supplement Plan N may be an excellent alternative when its monthly premium is meaningfully lower. Both plans require you to pay the annual Medicare Part B deductible ($283 in 2026).
With Plan N, you may also pay up to $20 for certain office visits, up to $50 for emergency room visits that do not result in an inpatient admission, and applicable Medicare Part B excess charges.
The important question is whether Plan N’s premium savings outweigh its potential copays and any excess charges. Since most Medicare clinicians accept assignment, Plan N can offer meaningful savings for many people. We compare premiums, check whether your preferred providers accept assignment and review your anticipated healthcare use rather than automatically recommending the plan with the most benefits.

Changing Plans (California “Birthday Rule”)
If you are concerned about qualifying medically for a plan change, California’s Birthday Rule may allow eligible Medicare Supplement policyholders to switch to a plan with equal or lesser benefits without medical underwriting during their annual birthday enrollment window.
Important: Prescription Drug Coverage
Original Medicare and Medicare Supplements do not include outpatient prescription drug coverage. Most people also enroll in a standalone Medicare Part D Prescription Drug Plan (PDP) to help cover their medications.
